We offer three options for our Handlers to fill out the Assessement Transmittal Report. Please view your options below:

In lieu of this form, a computer generated report containing the required information may be submitted including a signed statement.

When Uploading your Report, please be sure that they include the Handler Name, Address, quarter and year as well as the following information for each Producer:
Producer Name and Address, Year Grown, County Grown, Lbs of Seed, Grass Species,
$ Amount of Trasaction and $ Amount of Assessment
